I had done this in the past but it appears to have become unlinked. The only way to reach it from a ClearSea extension is to type in the public IP address.

Also, can you clarify if you are looking for a general explanation and possible options to register a 220 endpoint on a ClearSea server, as your title would suggest, or are you trying to troubleshoot a specific issue, where registration used to work, and then stopped working, as the content would suggest?
